Summary

Position Summary

The Lung Cancer Screening Navigator will focus on patient navigation through the lung cancer screening continuum. On an aggregate level, the Lung Cancer Screening Navigator participates in a collaborative process of assessment, care coordination, planning, facilitation, referral, and advocacy for options and services to meet an individual’s health needs. This position will track and follow up with patients identified through the lung screening program, lung nodule clinic, and incidental lung abnormalities. Works with internal and external providers to ensure accurate orders, follow-up, and plan of care. Must be flexible and adapt to changes in the work environment; Must be able to work well in a team setting and manage complex demands. Take responsibility; keep commitments and complete tasks on time.

 

Minimum Requirements

Education

  • BSN

Experience

  • 3+ years nursing experience
  • Knowledge of adult learning principles, Public Speaking, must be able to interact and relate to populations served

License/Registration/Certifications             

  • Valid driver’s license, RN

Preferred Education

  • MSN

Preferred Experience

  • 5+ years outpatient nursing experience
  • Pulmonology experience
  • Bilingual

Core Job Responsibilities

  • Responsible for the navigation, follow-up, and education of patients seen in the lung cancer screening program and identified patients with incidental lung findings.
  • Reviews lung screening worklist daily to ensure patients meet criteria for lung cancer screening.
  • Facilitate the presentation of high-risk lung imaging (L4’s) at Thoracic MPC.
  • Develop relationships with radiology, pulmonology, oncology, and surgery to ensure seamless referral and patient disposition.
  • Assists the Lung Cancer Screening Manager with daily operations, marketing, outreach, provider education, and growth of the program.
  • Track patient progress through diagnostic and treatment processes through medical record review and follow-up with patients to ensure recommended services were received.
  • Assist with providing nursing assessment and intervention in the Lung Nodule Clinic.
  • Use and understanding of Epic (EHR) Lung Screening Dashboard.
  • Participate in weekly Thoracic MPC.
  • Works collaboratively with the Lung Cancer Nurse Practitioner, Thoracic Clinical Navigators, and Imaging Navigators.
  • Contributes to team effort by accomplishing results as needed.
  • Properly documents all communication in accordance with policies and procedures.
  • May perform other duties as assigned.
  • While performing the duties of this position, the employee is regularly required to talk or hear.
  • The employee frequently is required to use hands or finger, handle, or feel objects, tools, or controls.
  • The employee is occasionally required to stand, walk, sit, reach with hands and arms; climb or balance; and stoop, kneel, crouch, or crawl.
  • The employee may have exposure to load noise.
  • Must be able to sit for an extended period.
  • Must be able to walk/stand for extended period.
  • The employee must occasionally lift and/or move up to 25 pounds.
  • Specific vision abilities required by this position include close vision, distance vision, color vision, peripheral vision, and the ability to adjust focus.
