Overview

General: The Oncology Nurse Navigator is responsible for the coordination of patient care across the continuum within the physician's prescribed plan of care, national oncology guidelines, and within the scope of nursing practice. Educates and provides information to patients and caregivers to guide and facilitate understanding of treatment plans prescribed by licensed independent practitioners and/or within scope of nursing practice. Specific: The Oncology Nurse Navigator is a patient advocate who: 1.) Guides and supports patients/families throughout the cancer care and survivorship experience. 2.)  Oversees, directs, and provides holistic, culturally competent and evidence-based nursing care. 3.) Monitors patient outcomes and participates in quality improvement activities. 4.) Contributes to and collaborates with healthcare team members to positively impact patient outcomes and patient experiences. 5. Assists patients or their caregivers with developing scope of services with clearly stated goals and objectives.

Responsibilities

  • Serves as primary point of contact for patients and families.
  • Provides education related to multidisciplinary process, oncology diagnosis, treatment, procedures, cancer center support services and community resources.
  • Maintains contact with patient throughout trajectory of care at designated touch points.
  • Provides comprehensive assessment of the oncology patient, family and/or caregivers needs at pivotal medical visits across the continuum, assesses barriers to care and distress screening, refers to appropriate services (social worker, oncology mental health therapist, financial counseling etc.), disciplines and/multidisciplinary rounds and ensures patient follow through.
  • Educates the patient, family and/or caregiver regarding clinical trials and coordinates participation with Clinical Trial Team as required.
  • Provides education and information to the patient and family, helping to make the care seamless, continuous and comprehensive.
  • Initiates and documents patient teaching including family and significant others based on assessment of needs.
  • Responds to patient request for information regarding the disease process, expected side effects of treatment and community resources. Uses appropriate patient Education documentation modality. Supports the patient during difficult decision-making periods. Assists in coordination of end of life care for patient and family and provides emotional support.
  • Coordinates patient multidisciplinary appointments to ensure timeliness of care. This may include referrals to palliative care, survivorship services, supportive care, physician office visits, tests, procedures.
  • Monitors patients to ensure follow through.
  • Communicates any concerns to the appropriate physician(s).
  • Participates in the multidisciplinary tumor conference and with the multidisciplinary team monitors recommendations for appropriateness with NCCN or other national guidelines. Additionally, participates on relevant committees or clinical performance groups to enhance patient care and patient experience. Serves as a point of contact for referring physicians and their staff.
  • Solicits feedback from referring physicians and their staff, develops recommendations for improving clinic operations to better meet referring provider needs.
  • As needed, meets with physicians who are potential (or are already established) referral sources to provide information about the cancer program.
  • Participates in performance improvement activities to enhance the patient, family and / or caregiver experience across the continuum.
  • Tracks individual patient's progress along care continuum, identifies and addresses issues, and prepares timeliness of care metric to present to appropriate committees.
  • Develops and maintains collaborative relationships with local individuals, agencies and organizations that provide cancer education and support services for patients their families and/ or caregivers.
  • As needed, develops, in conjunction with support staff or organizations, support groups and education programs for the patient, their families and/ or caregivers.
  • Functions in an organized and time conscious manner.
  • Partners with patients, families and community resources to provide well-coordinated, timely, compassionate, exemplary, interdisciplinary care.
  • Communicates with all members of the healthcare team, as appropriate about patient/family needs and concerns. As requested, serves as resource for community educational events.
  • Completes charting in a timely manner.
  • Other duties as requested.

Qualifications

Three to four years of progressively more responsible clinical nursing experience in oncology preferred.

Bachelor's degree in Nursing required. MSN preferred.

Oncology Certified Nurse (ONS) or Nurse Navigator certificate within two (2) years of hire.
