Description
Primary responsibility is 24-hour on-call coverage for coordination of/liaison between transplant teams, patients, hospital departments, and organ procurement organizations for transplants. Secondary responsibilities encompass all aspects of clinical coordination of the transplant recipient involving all phases of the transplant process, from initial pre-transplant referral (including urgent transfers) throughout the post-transplant patient follow-up phase. This responsibility involves the coordination of the interdisciplinary medical care delivery system under the direction of the transplant program physicians, nurse manager, and nursing supervisors to facilitate liaison functions between the patient/family and the medical staff, referring physicians, and insurance companies. Additionally responsibilities include: triage of patient and family calls, coordination and workup of incoming new patient referrals that occur outside business hours, a working knowledge of UNOS data entry, and individualized patient/family teaching and assessment of appropriate medical intervention for effective use of resources and quality assurance. Transplant donor/recipient coordination and liaison function with organ procurement agencies. The Transplant Coordinator reports directly to the Nurse Manager. Clinical functions are supervised by the Transplant Program Medical Directors and the Nurse Manager. Overall job performance is evaluated jointly by the Transplant Program Medical Directors and the Nurse Manager. Salary range : $163,720 – $220, 096 annually
