Staff Nurse

Harbor Health is seeking per diem Staff Nurses to provide as needed coverage at our health center in Dorchester, Geiger Gibson Community Health Center with ability to float to our other centers including Daniel Driscoll Neponset Health Center as needed.
Role:
The Staff Nurse provides nursing services at the top of the incumbent’s license to patients in accordance with all applicable Massachusetts State Laws as well as regulations promulgated by the Department of Public Health and Harbor Health Services, Inc. Duties include but are not limited to triage; access; care coordination and nursing support of clinical sessions.
Responsibilities:
- Performs all nursing duties in accordance with Massachusetts Nursing Practice Act, while adhering to Harbor Health policies and procedures.
- Performs routine nursing care to patients, while utilizing age specific guidelines and standard precaution practices at all times. Performs comprehensive assessment of patients health status and triage based on urgency of symptoms, in person and via telephone.
- Performs patient teaching for all patients and families.
- Actively demonstrates a working knowledge of nursing theory, techniques, principles, and practice to holistically care for the patient.
- Provides patient education on disease prevention, medications and treatment plans and self-care techniques.
- Acts as liaison between the patient, the care team and/or provider.
- Provides health coaching and teaching for common conditions (i.e. asthma, hypertension, diabetes) through individual nurse visits, group visits and co-visits with other disciplines as appropriate.
- Helps patients stay up to date with clinical care guidelines, immunizations and health maintenance screenings.
- Administers medications and vaccinations to patients as ordered by provider or by following standing orders policy.
- Assesses information about the patient’s physiological, psychological, sociological, and spiritual status.
- Promotes health and wellness through education and preventative care and counsels patients on lifestyle modifications as needed.
- Documents patient care activities, assessments, treatment plans and interventions in EHR, ensuring accurate and timely documentation of patient encounters.
- Serves as an advocate for patients, ensuring their needs and concerns are addressed.
- Supports patients in navigating the healthcare system and understanding their patient rights and options.
- Demonstrates knowledge of medical equipment and appropriate usage.
- Complies with all facility policies and procedures, while upholding the high expectations of Harbor Health Services, Inc.
- Maintains knowledge of all medications, reactions, documentation, and dosage requirements.
- Certifies that emergency equipment in clinical area is appropriately maintained and in working order.
- Follows HHSI policy when disposing of biohazardous materials, labeling and handling all materials.
- Participates in team, department, and clinic meetings to learn, support process improvement, and contribute to care plans as appropriate.
Requirements:
- Successful completion of a nursing program (RN), BSN preferred
- Current MA RN License, current Basic Life Support (BLS) certification
- Experience in a community health setting preferred
- Ambulatory care or health insurance environment experience preferred
- Basic computer literacy, knowledge and experience with Microsoft Office 365 including Word, Excel, Outlook and Teams, knowledge of Electronic Medical Record (EMR), EPIC/OCHIN EMR preferred.
- Excellent customer service skills and verbal and written communications skills
- Ability to effectively work within both a team setting and independently
- Ability to work effectively with economically, racially and linguistically diverse customers
- Bilingual a plus, English and Vietnamese/Spanish/Portuguese or Haitian Creole

Our health centers are open 1 or 2 evenings a week (Tuesdays and Thursdays) and Saturdays, 8:30 AM - 12:30 PM at Neponset Health Center.
Shifts are 8:00 AM - 5:30 PM or 10:45 AM - 8:30 PM Monday through Friday availability
Per Diem (on call) as needed to cover open shifts
