Harbor Health also operates two Programs for All-Inclusive Care for the Elderly (PACE) in Mattapan and Brockton, providing comprehensive healthcare, transportation and social services for more than 580 frail elders who continue to live with dignity and independence in the community and a Woman, Infants and Children (WIC) Nutrition Program.

Role and Responsibilities:

The ESP Senior Medical Director provides leadership and management for the clinical staff of the Elder Service Plan - Program for All Inclusive Care for the Elderly (PACE) Centers as well as provides primary care, including routine health maintenance, management of chronic conditions, and management of acute illnesses.

Duties include but are not limited to: the hiring of and the direct supervision of medical providers; assisting in the development, implementation and management of the annual visit budget; assisting with the development, implementation and evaluation of clinical and related policies and procedures for the PACE Centers and Harbor-wide community; participating in Harbor-wide quality assurance efforts; assisting and actively participating in any health care audits conducted by the Dept. of Public Health (DPH), Centers for Medicare & Medicaid Services (CMS) and other compliance and regulatory agencies;  participating in strategic planning discussions with the Senior Leadership Team (SLT); implementing agency mission and ensuring patient focused approach in operations of Harbor’s PACE Centers.

Provider Relations

  • Recruits, supervises, and evaluates all primary care and specialist provider staff-physicians and nurse practitioners.
  • Provides orientation for all newly recruited provider staff.
  • Responsible for the provider training plan including the orientation plan for new providers and training in coding and charting.
  • Works collaboratively with the Director of Homecare and Site Director on the development of an effective partnership between provider and nursing staff.
  • Represents provider perspective in planning for program changes, including growth opportunities and expansion.
  • Responsible for the maintenance, communication, and improvements to provider contracts and compensation plan.

Administrative

  • Assists Human Resources Dept. with provider privileging and credentialing.
  • In collaboration with operational and clinical leadership, assumes a responsible role in operational improvement and problem solving in PACE center operations, including clinical care, access, utilization management, staffing, patient scheduling and financial issues.
  • Works with SLT on strategic planning, implementing agency mission and ensuring patient focused approach in operations of the PACE Center.
  • Oversees medical management process in the centers, working under the guidance of agency medical management approach.

Patient Safety and Quality 

  • Responsible for oversight of Clinical Quality for PACE.
  • Assists in the development and implementation of quality improvement plan and efforts including peer review, meeting CMS standards and other external review processes.
  • Responds promptly to staff and patient complaints about standards of practice and quality of care.
  • Oversees provider peer review process.
  • Develops and ensures timely feedback and coaching for all providers on measures of clinical and patient experience performance.
  • Responsible for the oversight of Clinical Risk Management activities
  • Advises CEO and compliance leadership on risk management activities, including any malpractice claims.

Community Relations

  • Collaborates with other providers, including PACE organizations, community mental health centers, social service agencies and area hospitals/provider networks.
  • Participates in the development of clinical affiliations with local area hospitals and provider networks.
  • During emergency operations may be required to report to work or remain at work.

Requirements: 

  • Completed accredited physician residency in Internal Medicine or Family Practice with additional training in Geriatric Medicine
  • Current Massachusetts MD or DO license, Board Certified, Current DEA, MCSR and Basic Life Support Certification required
  • Minimum of  5 years of primary care experience
  • Experience in a community health care center preferred
  • 3-5 years of administrative and/or leadership experience
  • Experience in the development and implementation of geriatric programs
  • Strong clinical, leadership, interpersonal and organizational skills
  • Experience working with a multidisciplinary team
  • Knowledge of community resources preferred
  • Must be able to travel timely between Elder Service Plan and partnership sites and diagnose and triage on site or client’s home
