Job Requirements
Position SummaryCardiovascular Outpatient RN for Cardiac and Pulmonary Rehab: The Registered Nurse, under the direction of the Manager, assesses, develops and implements plan of care given as well as the response measured from the exercise prescription. Additional responsibilities include patient education concerning cardiac and pulmonary risk factor reduction education, activity progression and dietary modification for adults 18 and older.

Minimum Requirements
Education
- Graduate of an accredited nursing program
- Associates Degree in Nursing
Experience
- Minimum of 2 years cardiac experience
License/Registration/Certifications
- Current license to practice Nursing in the State of South Carolina
- Certification in Basic Life Support
- Cardiac Rehab and Outpatient Heart and Vascular Center only: Advanced Cardiac Life Support
Preferred Requirements
Preferred Education
- Bachelor's Degree in Nursing
Preferred Experience
- Cardiac Rehab: 1 year of Cardiac Rehab experience
- Heart Failure Clinic: 3 or more years cardiac experience
- Outpatient Heart and Vascular Center: 2 years of cardiac experience
Preferred License/Registration/Certifications
- Cardiac Rehab: Certified Cardiac Rehab Professional
- Heart Failure Clinic: Certified Heart Failure Nurse Certification
Core Job Responsibilities
- Collaborates with patients, families, and other members of the health care team to determine patient priorities and treatments
- Initiates and facilitates interdisciplinary collaboration to enhance outcome
- Seeks necessary assistance in completing nursing interventions to assure positive patient outcomes
- Responsible for demonstrating competency for all new equipment and procedures.
- Provides clinical guidance to students and ancillary staff
- Successfully completes hospital and department-based orientation programs
- Formulates a plan for professional growth
- Ensures compliance with all clinical expectations regarding regulatory requirements
- Identifies appropriate standards of care, and/or collaborative clinical pathways to guide the plan of care
- Complies with established policies and procedures
- Identifies the need to develop or revise policies and procedures pertaining to specialized patient populations and/or unit and hospital-based practices
- Serves on hospital-based committees
- Assesses assigned patients' medical history, admitting diagnosis and physician's orders, obtaining nursing history, performing physical assessment, evaluating diagnostic data, and interviewing patient and family to determine patient needs
- Demonstrates knowledge of professional nursing practice and theory.
- Assures continuity of care through effective decision making resulting in quality patient care
- Documents all aspects of patient care by following established charting procedure to show the patient's progress or any change in the patient's condition
- Consistently follows all regulatory guidelines for the department and hospital.
- Completes all documentation requirements thoroughly and in a timely manner per hospital requirements
- Communicates effectively with all members of the care delivery team.
- Responsible for participating in process improvement projects within the department.
- Provides excellent customer service and effective communication with both internal and external customers of the department.
- Serves as a clinical resource within the department.
- Ensures that appropriate critical path/standard of care/plan of care is identified and collaboratively implemented.
- Adhere to and practice appropriate infection control policies and procedures.
- Perform accurate, legal, and ethical documentation at all times.
