Summary

Position Summary

 

Primary responsibilities for program outreach are to bring and maintain patients into care; provide in- hospital and EC patient outreach; coordinate with care partners to encourage patient testing. Specifically, the Outreach worker will identify and positively impact: patients who did not return for follow-up after intake or were diagnosed at health department/inpatient admission and did not enter care; and patients who have been lost to follow up, to find out if they are in care elsewhere or reestablish medical care in our program; track outside consultation appointments and ensure patients are able to be compliant with their plan of care. The Outreach worker will also assist in adherence to follow up appointments in the office.

 

Minimum Requirements

 

Education

  • High School Graduate or Equivalent

 

Experience

  • Five years physician practice experience

 

License/Registration/Certifications       

  • Reliable Transportation with proof of insurance
  • Current SC Driver’s License with safe driving record

Core Job Responsibilities

 

  • Conduct initial comprehensive patient intake to include all necessary documentation in the EHS system.
  • Conduct all programmatic outreach activities and targeted education including street outreach, detention centers, shelters, health fairs, soup kitchens, clubs and other venues where at risk individuals are known to gather.
  • Provides Ryan White program information to community and conduct community-based program intakes when necessary.
  • Link outreached persons known positive to Ryan White, primary care, mental health, nutrition, and other services that are available.
  • Support Ryan White Coordinator/Case Managers/Nurses in the coordination of services for PLWHA.  Outreach to non-responsive to phone or written contacts by MCM, outreach will attempt to make a home visit to reconnect patient with care.
  • Assess persons with unknown HIV status readiness to be tested.
  • Collect and complete all necessary paperwork required.
  • Actively collaborates with other departments to develop mutually supportive programs.
  • Participate outreach events.
  • Submit reports regularly regarding education and outreach activities to the EIS Project Director.
  • Works autonomously with the community and closely with the case managers, nurses and others to ensure care.
  • Perform other duties as assigned.
  • Must be in good physical condition and able to withstand the stress and demands of an active position.
  • Manual dexterity, good eye/hand coordination and acceptable vision needed in daily work.
  • Must have strong knowledge of organizational dynamics and have excellent communication/coaching skills.
  • Must be able to establish and maintain effective working relationships with patients, employees, the public, and other agencies.
  • Must be able to respond calmly and efficiently in crisis situations.
  • Effective verbal and written communication skills based on the command of the English language.
  • Must be able to perform the following functions:  standing for long hours, walking, stooping, bending, sitting, pushing, pulling, and squatting.
  • Must be able to lift 30+ lbs. in order to lift/move patients, equipment, and/or supplies utilized in the work place.
