Clinical Staff and Ambulatory Pharmacist
Job Description
POSITION SUMMARY
The Clinical Staff/Ambulatory Pharmacist provides comprehensive patient care services through inpatient pharmacy functions, including but not limited to order entry/verification, medication checking/dispensing and other pertinent tasks that ensure safe and effective use of medications. The Pharmacist may also rotate into ambulatory roles such as anticoagulation clinic services, oncology or others as determined by Manager.
The Clinical Staff/Ambulatory Pharmacist will be involved in patient care activities supporting a variety of patient populations, ranging from geriatrics to pediatrics.
MINIMUM REQUIREMENTS
Education:
- Bachelor of Science in Pharmacy required.
- Doctor of Pharmacy degree preferred.
Experience:
- Recent hospital experience preferred.
- If rotating to an Ambulatory position:
- Pharmacy residency experience preferred.
- Advanced practice certifications preferred
Knowledge, Skills, Abilities:
- Knowledge of all current state and federal laws pertaining to the practice of pharmacy
- Ability to positively interact with other pharmacy technicians, pharmacists, providers, nurses, other health system employees, and medical sales representatives.
License/Certification/Registration:
- Current license issued by the Pennsylvania State Board of Pharmacy or eligible for reciprocation in Pennsylvania within 90 days of hire.
- If rotating to the Anticoagulation Clinic:
- Meets the American Heart Association’s standards for Healthcare Provider Basic Life Support (BLS) and renews BLS every two years.
SUPERVISION RECEIVED
Receives minimal supervision from the Pharmacy Manager and Supervisors.
SUPERVISION GIVEN
Supervises the activities of the Pharmacy Technicians and Graduate Pharmacy Externs
Responsibilities
ESSENTIAL FUNCTIONS
- Provides comprehensive central pharmacy services as follows:
- Appropriately enters and/or reviews medication orders for patients.
- Dispenses medications in an accurate and timely manner.
- Provides drug information in an accurate and timely manner.
- Documents monitoring and evaluation through the pharmacy computer system and clinical surveillance program.
- Assists with overseeing day to day operations in the sterile compounding area, ensuring aseptic and accurate production of compounded sterile preparations.
- Prepares high-risk drugs.
- Prepares cytotoxic chemotherapy drugs.
- Prepares narcotic infusions.
- Performs appropriate drug therapy monitoring per protocol
- Performs pharmacist-managed consults per protocol, as assigned.
- Demonstrate continuous professional development by maintaining a portfolio of items of interest for educational and professional growth interests.
- If rotating to an ambulatory role will facilitate and document drug therapy decisions via recommendations and/or collaborative practice agreements.
- Performs supervisory duties.
- Supports an environment of teamwork and cooperation to accomplish quality pharmaceutical patient care and departmental goals.
- Directly supervises all Pharmacy Technician and Graduate Pharmacy Extern positions.
- Assumes shift supervisory role in the absence of the Manager and Supervisors.
- Communicates effectively with others.
- Interacts with patients, providers, nurses, other health system personnel, and the community.
- Provides pertinent information during department meetings and shift reports.
- Promotes a positive work environment by providing constructive ideas and feedback to improve services and the department
- Utilizes available technology proficiently and to full extent, in their area of practice.
- Assists with inventory control.
- Promotes proper and efficient use of pharmaceuticals.
- Manages assigned departmental and other health system drug inventories.
- Conducts ongoing education and training.
- Provides medication use education to patients, providers, nurses, other health system personnel, and the community.
- Provides departmental pharmaceutical education programs.
- Acts as a training resource for all Pharmacists (as appropriate), Graduate Pharmacy Externs and Pharmacy Technician positions.
- Performs competency validation.
- Participates in and supports DUE, performance improvement and quality control activities.
- Supports the continuous review of the medication use process and departmental procedures.
- Identifies opportunities for improvement.
- Participates in monitoring activities and data analysis.
- Assists with policy review, as applicable
- Follows rules and regulations.
- Maintains a working knowledge of all laws pertaining to the practice of pharmacy.
- Fulfills a pharmacist's duty to uphold all rules and regulations.
- Controls ordering, dispensing, and distribution of all controlled substances.
- Complies with the safety program in accordance with health system policies.
- Attends assigned safety training sessions.
- Complies with health system safety policies and encourages persons under his/her direction to do the same.
- Documents attendance at required training on Staff Development/Education and Training Record.
NON-ESSENTIAL FUNCTIONS
Performs related and miscellaneous duties as assigned.
