Job Description

JOB SUMMARY: Provides care coordination and discharge planning services for all inpatients accessing care through the UM Upper Chesapeake Health and Harford Memorial. This position will complete psychosocial assessments, assist with treatment planning, monitoring patient progress, facilitating patient and family meetings, and coordinating and implementing discharge services. Screens patients to identify anticipated needs, interacts with patients and families, so that a safe and timely care plan is achieved. Coordinates and implements discharge and post-acute services for inpatient caseload. Education & Training: Current Maryland RN license required.  Completion of a Bachelor’s of Science degree in Nursing preferred. Masters of Nursing also preferred.   Work Orientation & Experience: Acute care experience required.  Experience in Case Management, Utilization Management and Critical Pathways preferred.  Basic computer skills required
