Physical Therapist-Home Health
Description
Under the direction of the Rehab Director/Manager, the Physical Therapist (PT) is responsible for assessing, evaluating, planning and implementing a medically prescribed treatment plan established for patients in the homecare setting to improve, restore, and compensate for lost function and to facilitate achieving independence for their optimal physical ability. The PT is responsible for modifying treatment plans and procedures and using appropriate communication techniques for specific age groups. The PT applies knowledge of growth and development and the aging process to achieve optimal results. The PT supervises PTA and Home Health Aides according to South Carolina state regulations. Other responsibilities will be assigned as appropriate.
Minimum Requirements
Education
• Baccalaureate or Master's Degree in Physical Therapy. Graduate of physical therapy program
approved by the Commission on Accreditation in Physical Therapy Education (CAPTE) or
successor organizations of CAPTE
License/Registration/Certifications
• Current license to practice as a Physical Therapist in South Carolina
• Valid SC driver's license with good driving record
• CPR certification

• Prefer one (1) year of clinical experience.
Core Job Responsibilities
• Plan and administer prescribed skilled physical therapy treatment and training for patients suffering from various injuries, illnesses, and functional disabilities to attain highest level of physical function.
• Test/screen the patient's physical strengths to assist the physician in evaluating the patient's level of function and records findings to develop or pursue treatment programs and establish measurable training objectives.
• Develop/implement a conditioning/rehabilitation program consistent with physician's Plan of Care and the overall measurable goals of the patient/rehab team.
• Adjust treatment as needed to achieve maximum results. Confer with physician and clinical team members to obtain additional patient information and assist in developing, implementing, and revising the therapy treatment program and Plan of Treatment.
• Evaluate effects of physical therapy treatments by observing and evaluating patient's progress; recommending adjustments and modifications.
• Provide Physical Therapy Assistants and Home Health Aide staff with written care plan that reflects current plan of care as related to therapy, supervise/evaluate staffs' performance.
- Assures continuation of therapeutic plan following discharge by designing home exercise programs; instructing patients, families, and caregivers in home exercise programs; recommending and/or providing assistive equipment; recommending outpatient or home health follow-up programs.
