Shift:  Day

Working Hours:  8-4:30

Summary:
Responsible for supervision of all staff taking referrals for Home Health and Hospice. Manage processes to obtain accurate, complete information for clinical staff to start/resume patient care. Responsible for obtaining face-to-face documentation from appropriate physician.

Other information:
(1) Registered Nurse, Physical Therapist, Occupational Therapist, or Speech Therapist with current Arkansas license.
(2) Three years of clinical experience
(3) One year of Home Health/Hospice supervisory experience preferred.

This job will be authorized 80.00 hours bi-weekly.
