POSITION SUMMARY

Performs a variety of procedures necessary in the diversified duties of outpatient Physical Therapy from the physicians order and general instruction. The Physical Therapist actively participates as a member of the healthcare team to engage patients, families, and caregivers in the coordination of patient care in a clinical setting or in patient’s homes (mobile environment).

MINIMUM REQUIREMENTS

Education:

  1. Graduate of an approved program in Physical Therapy

Experience:

  1. One year of direct patient care in an outpatient setting is preferred.
  2. Experience in mobile outpatient physical therapy is preferred.

Knowledge, Skills, Abilities:

  1. Must be able to perform all duties associated with the position of Physical Therapist both in a clinical and in a mobile environment.
  2. Good physical condition is necessary to endure stresses of the job.
  3. Strong communication skills and interpersonal skills.
  4. Must display professional mannerism and appearance.
  5. Demonstrates maturity to function independently, organize work and set priorities appropriately.
  6. Ability to effectively deal with emotional and medical needs of patients.
  7. Ability to communicate verbally via telephone.
  8. Ability to use good judgment and remain calm in high-stress situations.
  9. Ability to schedule patients and collaborate with scheduling staff to ensure continuity of care.
  10. Ability to read and follow road maps; and ability to use a Global Positioning System (GPS) mapping system.
  11. Has access to reliable transportation.

License/Certification/Registration:

  1. Must be a licensed Physical Therapist or eligible for licensure in the state of Pennsylvania.
    1. Possession of or eligible for a PA Direct Access License preferred.
  2. Valid Driver’s license.
  3. Meets the American Heart Association’s standards for Healthcare Provider Basic Life Support (BLS) and renews BLS every two years.

SUPERVISION RECEIVED

Receives minimal supervision from the practice manager.

SUPERVISION GIVEN

Oversees the clinical activities of support staff when working in clinic.

Responsibilities

ESSENTIAL FUNCTIONS

  1. Provides direct patient care while effectively building rapport and trust with the patients.
  2. Performs responsibilities in a manner which demonstrates professionalism.
  3. Establishes an effective and appropriate physical therapy plan of care based on diagnosis and prescription in collaboration with physicians and contracting agencies.
  4. Executes the plan of are and measures it at appropriate intervals.
  5. Consistently demonstrates flexibility to adjust the plan of care according to the patients’ best long term health care needs.
  6. Works effectively with support staff (Physical Therapy Assistants and Technicians) to progress through their plan of care.
  7. Encourages patients to complete the full course of therapy without discontinuing the plan of care
  8. Provide patients with effective home exercise programs (HEPs).
  9. Educates patients about benefits of fitness services provided by Fit for Play.
  10. Maintains competency through self-study, peer review, and continuing education.
  11. Understands proper documentation processes for compliance and billing purposes.
  12. Completes all paperwork related to patient care.
  13. Helps to maintain pre-established inventory of equipment and supplies.
    1. Performs a check of patient care supplies on each shift and restock any supplies that are missing or in need of repair or maintenance.
    2. Notifies supervisor of faulty equipment as soon as noted (verbal, telephone or written).
    3. Proactively notifies supervisor of equipment that requires purchase or restocking.
  14. When required, completes proper documentation related to travel in the required time frame.
  15. Consults with and keeps supervisor informed of needs and problems.
    1. Reports unusual events / safety issues to supervisor in accordance with program policies.

NON-ESSENTIAL FUNCTIONS

Performs related and miscellaneous duties as assigned.
