Summary
Position Summary
The Licensed Practical Nurse provides safe and effective nursing care that requires knowledge, judgment, and skill and is based on current education level and competence, rendered directly under the supervision of the Registered Nurse.
Unit Description
2 West is our 21-bed step down unit within Spartanburg Hospital For Restorative Care, a long term acute-care hospital (LTACH). We treat a wide variety of patients with various conditions needing longer term acute care. Our average patient stay is 28-30 days. This is an exciting area to work in for someone who enjoys the acute care setting with extensive variety, while building patient relationships, and having a close-knit, family-like working environment centered around teamwork.
Our long-term, acute care (LTACH) unit focuses on patients with serious medical problems that require special treatment.
Successful intervention requires a comprehensive approach involving often involving numerous professions and technologies. Educating the patient and family is crucial to achieving success and breaking the cycle of readmission. Patients reach their optimal and realistic goals through work, family involvement, and intervention of many professionals.
Our team provides individuals with services on the following conditions:
- Pulmonary disease
- Cardiac disease
- Infectious disease
- Medically complex
- Post trauma
- Renal disease
- Wound care
Minimum Requirements
Education
- Diploma in Applied Science with a major in Practical Nursing
- Graduate of an Accredited Nursing Program
Experience
- N/A
License/Registration/Certifications
- South Carolina LPN Nursing License
- Current BLS certification

Core Job Responsibilities
Assist with the collection of ongoing health care data; recognizes existing relationships between such data to normal values and the patient’s current health status

- Assist with the collection of patient tasks to support the patient’s health information and history
- Assures continuity and quality patient care through effective communication
- Collaborates with patients, families, and other health care providers in the delivery of health care
- Provides input that supports and contributes to the registered nurse’s assessment, analysis, and formulation of a nursing care plan
- Demonstrates knowledge of medications and correct administration
- Procures resources necessary to implement the nursing care plan and implements nursing interventions and tasks as outlined in the nursing care plan and as delegated by the registered nurse
- Seeks necessary assistance in completing nursing interventions to assure positive patient outcomes
- Documents all aspects of patient care rendered by following established electronic health record documentation policies and procedures.
- Participates in hourly rounding and bedside shift report
- Responsible for demonstrating competencies for all new equipment and procedures
- Successfully completes hospital and unit-based orientation programs
- Formulates a plan for professional growth
- Ensures compliance with all clinical expectations regarding regulatory requirements
- Participates as an effective and collaborative team member
- Other Duties, as assigned, under the supervision of a Registered Nurse
