  • SUMMARY OF RESPONSIBILITY - Job Summary (general statement of purpose or objective of position)
    • Baptist Health Home Health/Hospice RN Case Manager provides, manages and coordinates care and services for an assigned caseload of patients.
  • MINIMUM REQUIREMENTS - Knowledge, skills, abilities, license, registration, certification, education, and experience
    • Education required: Graduation from an approved program of Nursing.
      Required: Current Arkansas RN License.
      Required: Current Arkansas Driver's License.
      Preferred: Minimum 1 year of nursing experience. Home Health/Hospice experience preferred.
