General Summary
The Care Manager collaborates with members of the healthcare team, community resources, and with the patient/patient family to promote optimal outcomes for the patient across the continuum of care. The Care Manager provides Care Management activities to ensure an effective, appropriate, individualized and safe plan of care. The position is goal-oriented with a high level of accountability correlated to the continuum of care, quality of care and patient satisfaction.

Reporting Relationships
Reports to the Manager of Care Management.
What You Need to Be Successful:
Qualifications
Education: Graduate of an accredited college with a degree in social work and current licensure in the State of Maryland.
Master’s degree with LGSW or LCSW certification required.
Experience: Minimum of 3-5 years of relevant clinical hospital experience required as applicable to the position needs. Ongoing education, certification, and self-development to remain current with industry standards and business objectives related to hospital case management.

Hi, my name's Morgan. I started my nursing career a little over a year ago at UM BWMC on the progressive care unit. As a new nurse, I was extremely nervous to start, but PCU is such a strong, amazing team, and I felt supported and appreciated immediately. I learn so much from my patients and my colleagues every day, and I feel comfortable asking anyone on the floor anything. BWMC has provided me with the resources I need to be successful personally and successful in providing the highest quality of care to our community. When I started, I received all the necessary training, telemetry training, stroke-specific training, and I've become ACLS certified.
