Provides social work services, complex discharge planning, and care coordination to patients and their families. Identifies community services and refers patients in need to the appropriate service(s). Collaborates with other members of the multi-disciplinary team to affect positive patient and family outcomes. Intervenes with patients who have complex psychosocial needs, offers crisis intervention to patients and families with psychosocial needs, and coordinates and facilitates the development of a discharge plan of care for high-risk patient populations. Communicates with other hospital staff re: psychosocial issues that impact health condition/status.
EDUCATION / EXPERIENCE
Master's degree in Social Work from an accredited school by the Council on Social Work Education (CSWE) required. Social work experience in a healthcare setting preferred.
LICENSURE, CERTIFICATION, and/or REGISTRATION
Clinical licensure or provisional status preferred. Certification as Accredited Case Manager encouraged.

- Completes psychosocial assessment based on preliminary risk screening in accordance with departmental and professional standards to assess patient and family psychosocial risk factors through evaluation of prior functioning levels, adequacy of support systems, reaction to illness and ability to cope.
- Intervenes with patients and families regarding emotional, social, and financial consequences of illness and/or disability; accesses and mobilizes family/community resources to meet identified needs.
- Provides intervention in cases involving child abuse/neglect, domestic violence, elder abuse, institutional abuse and/or sexual assault.
- Serves as a resource person and provides counseling and intervention related to treatment decisions and end-of-life issues. Advocates for patient and family empowerment and independence to make autonomous health care decisions and access needed services within the health care system.
- Upon referral, collaborates with Case Managers though social work management of patient/family discharge needs. Addresses complex family dynamics that impact planning for patient care and discharge.
- Assures development of an individualized continuing care plan in collaboration with other team members.
- Collaborates and communicates with multidisciplinary team in all phases of discharge planning process.
- Collaborates/communicates with external case managers and/or external providers.
- Initiates and facilitates referrals for post-acute services.
- Facilitates safe and appropriate transfer to other facilities as appropriate.
- Promotes individual professional growth and development by meeting requirements for mandatory/continuing education, skills competency, supports department-based goals which contribute to the success of the organization; serves as preceptor, mentor, and resource to less experienced staff.
- Coordinates patient and family care conferences as needed.
- Documents in accordance with departmental reporting standards.
- Work in conjunction with Psychiatry to assure that the Involuntary Commitment (IVC) process is managed efficiently and effectively.
