Job Summary:

Reporting to the RN, Manager Palmetto SeniorCare PACE, the Homecare Supervisor is responsible for the development and implementation of homecare services for program participants. Includes the coordination of nursing, PT, OT and ST services as well as Durable Medical Equipment in the community.

Assesses, using the nursing process, the homecare needs of a frail elderly population, and identifies and develops specific plans of care.  Initial and periodic assessments to be conducted minimally every 6 months and must be completed prior to the scheduled team meeting.  Communicates participant changes with team members.

Coordination of 24 hour care delivery and the implementation of all homecare services which include nursing, PT, OT, ST and personal care services to ensure that quality services are provided to meet participant needs.

Coordinates and authorizes all Durable Medical Equipment.

Coordinates and supervises all nursing care for participants in the nursing home/boarding home according to program standards.  Includes interaction with other facility staff.

Manages human resources by: determining qualifications, competence and performance expectations; implementing and monitoring staffing plans; providing orientation, in-service training and continuing education of assigned staff and applying policies/procedures pertaining to human resources management.

Provides effective leadership by coordinating and integrating services within the department, with other departments and with the hospital's primary functions.

Performs other duties as assigned.


Education: Bachelor's degree in Nursing from an accredited school of Nursing program that is approved by the South Carolina State Board of Nursing required. Work experience may NOT substitute for education requirement.

Experience: At least 1 year as an RN with home or community health.  Supervisory experience desired.  Must have 1 year experience working with the frail or elderly.

Number and Type of Team Members Supervised: 1-5 Employees

Licensure, Registry or Certification Required: Licensed RN in SC.  Valid driver's license, an acceptable three-year motor vehicle record as defined by the Acceptable Motor Vehicle Record (MVR) Chart, and a certificate of auto insurance.

Special Training: Ability to work effectively in a team environment. Ability to treat geriatric patients. Driving is required for this position using the employee's personally owned vehicle.  Employee must have reliable transportation and will be expected to adhere to the Palmetto Health Driver Safety Policy and specific department driving policies.  Employee must pass driver training.
